New patient Information:
- Please arrive 10 minutes before your scheduled appointment to fill out paperwork and get registered.
- Wear comfortable clothing to make it easier to get to the area being treated if possible.
- Bring your referral for physical therapy from your physician to your first appointment.
- Bring your insurance card and any other important documents that may be useful to your treatment such as MRI’s, X-rays and Precautions.
- We participate with all insurance plans and will assist in verifying benefits prior to treatment.
Information you need to be aware of:
- Each treatment will be around 40-60 minutes.
- Please understand that regularly attending physical therapy treatments will be beneficial to your healing and well-being.
- BCPT will bill your insurance carrier however insurance co-payments are to be paid at the time services are rendered. Payment arrangements can be made with the billing department.
- You will need to attend all scheduled physical therapy treatment sessions.
- We ask you to notify us 24 hours in advance that you will not be able to attend your treatment. This consideration will allow for your scheduled appointment to be used for another patient.
- Missed appointments with less than 24 hour notice may be subject to a $25 fee. On the third absence, you will be discharged until you have the opportunity to revisit your referring physician.
